Mako System Certification Surgical Training Program Verona 1-2 December 2020
/Despite the complicated period due to the Covid19 pandemic the Clinica San Francesco ends the 2020 as protagonist in robotic surgery training.
The Drs, Piergiuseppe Perazzini (Director) and Paolo Sembenini, members of the Orthopaedics and Traumatology Functional Unit participated as speakers and faculty in the “Mako System Certification Surgical Training Program – Mako Total Knee & Partial Knee & Total Hip applications”, the certification course for robotic orthopaedic surgery held at the ICLO Teaching and Research Center of Verona (Italy) on Tuesday 1st and Wednesday 2nd December 2020. The training event was conducted by the Clinica San Francesco’s staff and represents now a recurring fixed reference point in the national orthopaedic landscape, aimed at surgeons from Italy who want to learn the robotic procedure.
The Perazzini’s staff represents the real European pioneer and outside the USA for the robotic prosthetic surgery, having introduced it in 2011 for the unicompartmental knee and in 2012 for the total hip (as well as being the first in Italy in 2017 for the total knee), having the widest and longest case history.
MALE PREVENTION. WHAT’S THE PSA (PROSTATE SPECIFIC ANTIGEN) TEST FOR?
/The PSA (Prostate specific antigen) is a protein produced by the prostate cells and is present in small quantities in blood. The test is carried out to investigate a possible thickening of prostate and then the presence of a possible gland disease as a cancer.
The prostate control must be carried out periodically by an urologist who, during the visit, reconstructs the clinical history of patient and examines the dimension and consistency of gland through a rectal exploration and, if necessary, prescribes the PSA dosage test.
The prostate problems could occur at any age as the prostatites that affect even the young people from the age of 20. The hypertrophy, thickening or cancer of prostate has a higher incidence from the age of 50.
It is clear that for those who have familiarity with the carcinoma of the prostate the prevention and so the PSA test must be carried out once a year and it is recommended from the age of 40, indeed, in absence of familiarity it is correct to conduct a control investigation of prostate every two years from the age of 50.
The Clinica San Francesco reaches 1000 robotic hip replacements
/The Clinica San Francesco consolidates its primacy in the field of robotic surgery.
In October 2020 MD Piergiuseppe Perazzini, Director of Orthopaedics and Traumatology Functional Unit, and his staff reached the number of 1000 hip replacements carried out with robotic procedure. Dr Perazzini is the European pioneer of this procedure, having introduced it first in Europe (and second one worldwide after USA) in December 2012, and currently in Italy he has got the highest number of hip replacement operations (further the knee ones) with the Mako robot. Dr Perazzini had learned and acquired the innovative method at the Robotic Surgery Center of Mako firm in Fort Lauderdale, and at the orthopaedic clinic – directed by MD Jon Dounchis, in Naples, both in Florida (USA).
The procedure minimizes the error margin in the prosthesis placement, main cause of instability and dislocations with frequent need of re-operation. This allows to achieve a high surgery precision, permitting a correct biomechanical working of implant. The expertise and long experience of Dr Perazzini made him the top expert of the field to which the Italian and European colleagues address to learn in detail the robotic procedure through technical laboratories, live surgery and lectures.
This allowed the Clinic to become in 2013 CORE (European Robotic Orthopaedic Centre) and in 2019 was recognized by the Veneto Region as reference facility for the robotic orthopaedic surgery.
WHY IS IT IMPORTANT THE FLU VACCINE?
/In this season the flu vaccine constitutes an important element of protection for persons considered more frail even more given the SARS-COV-2 health emergency.
The flu vaccine constitutes a protection, a simplification of the diagnosis and a better management of the Covid19 suspected cases, moreover it reduces the risk of complications related to the flu, and ensures a lower pressure on the health facilities also in terms of cost
Do Covid19 and flu virus are alike?
The World Health Organization has clarified what may be the similarities and possible differences between Covid19 and flu virus. Both appear with similar characteristics and attack the respiratory tract, and can be transmitted through contact, saliva droplets, sneezes or cough.
Although, if the range of symptoms seems to be similar, the disease impact on persons seems to be different. In this regard, the data – to the present day – concerning Covid19 suggest that the 80% of infections result soft and just the 15% of cases are severe, in flu cases the percentages related to serious and critical infections would be lower.
The Veneto Region is going to start the anti-flu campaign, the first region to get the supply of vaccines in early June 2020 with one million of doses. Also the Italian Health Ministry in the last few weeks reminded that “The aim is to enhance until the 75% the vaccine coverage, especially among the subjects at risk: persons from 60 years to up, children, chronically, immunodepressed, cancer and diabetic patients, and persons affcted by cardiovascular and respiratory diseases”.
International webinar with Professor Ibrahim Tuncay
/Although the complicated period due to the Covid19 pandemic, the Clinica San Francesco continues in the development of its intense international and academic relations.
On Friday 2nd October 2020, MD Piergiuseppe Perazzini, Director of the Orthopaedics and Traumatology Functional Unit, joined as speaker an important webinar dedicated to the hip robotic surgery with guest of honour the Professor Ibrahim Tuncay, Medicine, Orthopaedics and Traumatology Faculty of Bezmialem Vakif University, Istanbul (Turkey). During the event Dr. Perazzini provided the learned colleague his ten-year experience of robot use in operating room. The Professor Tuncay was delighted for the excellent results achieved from his first robotic hip replacement thanks also to the surgical approach developed by doctor Perazzini’s staff. The doctor Perazzini is the European pioneer (and the second one outside the USA) of the robotic orthopaedic surgery having performed first in 2011 a robotic partial knee replacement and in 2012 robotic total hip replacement, in addition to having introduced in Italy in 2017 the robotic total knee replacement.
The Clinica San Francesco today is an international reference point for robotic surgery, because it holds since 2013 the CORE (European Robotic Orthopaedic Centre) and in 2019 was recognized by the Veneto Region as reference facility for the robotic orthopaedic surgery.
ABDOMINAL SWELLING, CONSTIPATION AND METEORISM. IT COULD BE CAUSED BY “IRRITABLE COLON”
/Chronic pain, abdominal swelling, constipation, meteorism are just some symptoms of the irritable colon. Near 20% of Western countries people suffer it with a greater incidence among women.
It consists of a functional disorder of gastrointestinal system, a syndrome that does not show on sight alteration of intestine and of which the causes are not yet known. For this reason to diagnose become very difficult too. A specific diagnostic test does not exist, furthermore the symptoms may be attributed to many other pathologies which concern the intestine, so the only possible therapy is to care the symptoms everytime they occur.
The irritable colon syndrome is a chronic condition that can last over time, it’s possible to manage the disorder activating nutritional and psychological strategies, the latter such as to support the management of stress. Legumes, lactose and fructose are absolutely to avoid in order to prevent the increase of abdominal swelling. Also the consumption of gluten is to avoid for enhancing the symptomatology. Whereas the foods which may be consumed to purify the intestine and so limit the symptomatology are different. They are: apples, avocados, drinking a lot of water, leafy vegetables, fermented foods and apple vinegar.
Compared to other inflammatory diseases, as the Crohn’s disease, the irritable colon syndrome is not responsible for the change of intestinal anatomy so it may not be considered cause of cancer.
THE IMPORTANCE OF VACCINES AND FALSE MYTHS
/The Africa is finally free from poliomyelitis, the viral disease which affects the central nervous system and caused hundreds of thousands of paralysis and deformation cases among the children of the whole Continent.
The news was given by Tedros Adhanom Ghebreyesus, WHO Director-General on last 25th August. This historical milestone has been possible thanks to the synergical work of WHO and the Africa Regional Certification Commission (Arcc) that supervised and verified the eradication of the disease in the different African countries through the vaccination campaigns which allowed to obtain a coverage over 95% of population.
However, today the efficacy of vaccines and the importance of vaccination are questioned by some people.
Now, more than ever before, due to the health emergency of virus Sars-CoV-2, it is increasingly important to inform. The false myths around the vaccines are many and in this respect the Italian Higher Institute of Health dedicated on its website an entire paragraph.
THE CLINICA SAN FRANCESCO INTRODUCES THE CASE MANAGER
/The scientific progress and technological innovation brought the medicine to make great strides, the artificial intelligence has already in all respects entered as protagonist in the health care. This may have contributed to dishumanize the treatment, where the patient increasingly has been considered as an object and the relation between physician and patient widened its gap. Nowadays we talk more and more about “humanization of care”. It means taking care of patient not just from the point of view of disease, but also protecting the psychological and relational aspects. For this within many facilities, as at the Clinica San Francesco, has been introduced the position of Case Manager. The Case Manager has the task of accompanying the patient during all the therapeutic course to ease and coordinate the clinic procedure, attends to connect the different wards that come into play to carry out the healing process.

The Case Manager manages the care of every patient coordinating the treatment modalities and the necessary operations, plays a strategic role in the multidisciplinary team, for which translates into operational terms the objectives of treatment, and the sequence of treatment phases. The Case Manager is able to ensure the continuity of care, following the patient in all phases of course and making available to everyone who is taking care of the patient all information on his/her state of health, guaranteeing a totally personalized care.

THE REHABILITATION ROBOT – A STUDY OF THE CLINICA SAN FRANCESCO
/The robotic procedure is protagonist at the Clinica San Francesco, not just for surgery, but also for the rehabilitation. The staff directed by MD Piergiuseppe Perazzini realized a study on 12 patients who underwent an arthroplasty operation.
The group of patients was placed in an experimental protocol including robotic training and assessment sessions. The outcomes were compared with the another group ones, even this composed of 12 patients of the same age. This last though was under robotic assessment and treated with a traditional rehabilitation course. The preliminary results were promising, indeed, they showed the validity of training with the robotic platform in combination with the conventional therapy in intensive rehabilitation process after robotic knee arthroplasty.
This preliminary study will become object of scientific report followed by another research which will involve an increasing number of patients. The new study will use further assessment parameters thanks to the introduction of a new protocol called “Knee Index”.
